[Link] We are conducting a survey to better understand the awareness of services and needs of families and people served in the transition-age youth phase of life (ages 14-22) that are living in Tuscarawas County. Please fill out the survey if you are in one of the following categories:
You or a family member utilize TuscBDD transition-age services (or will utilize them soon) and you live in Tuscarawas County. For example, you are 13 and getting ready to enter this phase of life.
You are a person with a developmental disability between the ages of 14-22, and you live in Tuscarawas County.
You know someone in this age category that receives TuscBDD/Starlight School services, and you are willing to help them complete the survey (and the person lives in Tuscarawas County).
You are a parent/guardian of someone in this phase of life that receives TuscBDD/Starlight School services (and you live in Tuscarawas County).
You are a provider of services in Tuscarawas County for people with developmental disabilities in this age group.
You are an educational professional working in Tuscarawas County with people with developmental disabilities in this age group
